Credentialing and Privileging
What is required for each application process
Enrollment Application
The Managed Care coordinators will meet with you via Zoom to collect all information and documentation for your insurance/health plan enrollment.

Required Documents
The documents you have submitted to the Cottage Medical Group (CMG) generalist will transmit copies to the Managed Care team.

Further follow-up may be required for additional documentation.



















Privileges Application
The medical staff coordinator will send you a separate application for hospital privileges via MD-App.

The Managed Care team will help pre-fill the MD-APP hospital privilege application using any information you have provided.

Required Documents
The documents you have submitted to the Cottage Medical Group (CMG) generalist will transmit copies to the Medical Staff Office.

Additional privileging documents will be required. Please upload these to your MD-APP application.

Privileging
Select the privileges you wish to request via your online MD-APP application.

Be sure to review privileging requirements carefully, as additional documentation may be necessary to support your privileges request.

Review & Approval Process
After your full application is received, your application must be reviewed by medical executive leadership according to our monthly meeting schedule (link).

*Further information may be requested by leadership after you have submitted your completed application.

Employee Health Screenings
What to bring and what to expect at each visit
Appointment
A Cottage Health Talent Acquisition Business Partner will contact
you to schedule an appointment for a physical and urine drug screening.

Pre-Registration Paperwork
You will receive an email with instructions on how to complete your pre-registration physical paperwork based on the location you selected.

Please allow 7-10 business days for results.

*A preemployment drug screen is required for all positions. Applicants who test positive for a controlled substance, including marijuana are not eligible for employment.






Appointment
You will be scheduled to meet with Employee Health & Safety during your first week of hire.

TB screening
Required for all employees. TB history will be reviewed and further testing may be recommended. 

Immune Status Screening
Email your immunization records in advance to [email protected]
if you have documentation of prior vaccination or antibody titers.

Employee Health will review your immune status of measles, mumps, rubella (MMR), varicella (Chickenpox), Hepatitis B, TDAP, and seasonal influenza. Employees who do not have sufficient documentation of immunity may be advised to receive the appropriate vaccination within 30 days of their start date.

*This is a separate appointment from the physical and urine drug test.

Employee Health Procedures by Work Location
All new hires must complete a physical and drug screen before the first day of employment.


On-Site Employee
Employees working at a Cottage Health location will complete their physical and drug screen at one of our local clinics listed below. In addition, an appointment with Employee Health & Safety will be required.
Relocating Employee
If you are relocating to the Tri-County area, please complete your physical and drug screen at an approved clinic near your current residence before moving. Plan to arrive a few days before your start date to complete your Employee Health screening appointment, which must be completed on a weekday.

**Parking is limited at Cottage Rehabilitation Hospital. Street parking is available in the surrounding neighborhood, please plan ahead and allow extra time to find parking before your appointment.

Employee Onboarding
Steps and resources for Cottage Medical Group team members
Complete Credentialing and Receive Hospital Privileges
Physicians and APPs are required to be credentialed and privileged by their start date. Start date could be impacted if hospital privileges are not granted by date of hire.*
Complete Assigned Journeys
You will receive a "Welcome to Cottage Health" email with details on how to complete your assigned Journeys online through "Cottage Compass." Please complete within a week of receiving.
License and Certification Verification
All required licenses and certifications need to submitted no later than the Wednesday before your orientation date. (Start date could be impacted if not completed.)
Health Screening
A list of local health care facilities will be made available for you to complete your physical and drug screen which is required for all positions. In addition, an employee health screening is required.*
