A Cottage Health Talent Acquisition Business Partner will contact
you to schedule an appointment for a physical and urine drug screening.
What to expect
Most appointments take approximately 60-90 minutes, but timing may vary based on individual requirements and clinic volume.
Pre-Registration Paperwork
You will receive an email with instructions on how to complete your pre-registration physical paperwork based on the location you selected.
If visiting a Cottage Health Urgent Care location, complete the health packet below for your position type. Your appointment confirmation email will list where to send the completed document.
Please allow 7-10 business days for results.
*A pre-employment urine drug screen is required for all positions. Applicants who test positive for a controlled substance, including marijuana, are not eligible for employment.

Appointment
If your pre-employment physical and drug screening were completed at a location other than Cottage Health Urgent Care, an Employee Health & Safety staff member will schedule this additional appointment for you.
TB screening
Required for all employees. TB history will be reviewed and further testing may be recommended.
Immune Status Screening
if you have documentation of prior vaccination or antibody titers.
Employee Health will review your immune status of measles, mumps, rubella (MMR), varicella (Chickenpox), Hepatitis B, TDAP, and seasonal influenza. Employees who do not have sufficient documentation of immunity may be advised to receive the appropriate vaccination within 30 days of their start date.
*This is a separate appointment from the physical and urine drug test.
