Appointment
A Cottage Health Talent Acquisition Business Partner will contact
you to schedule an appointment for a physical and urine drug screening.
What to expect
Most appointments take approximately 60-90 minutes, but timing may vary based on individual requirements and clinic volume.
Pre-Registration Paperwork
You will receive an email with instructions on how to complete your pre-registration physical paperwork based on the location you selected.
If visiting a Cottage Health Urgent Care location, complete the health packet below for your position type. Your appointment confirmation email will list where to send the completed document.
Please allow 7-10 business days for results.
*A pre-employment urine drug screen is required for all positions. Applicants who test positive for a controlled substance, including marijuana, are not eligible for employment.
Appointment
If your pre-employment physical and drug screening were completed at a location other than Cottage Health Urgent Care, an Employee Health & Safety staff member will schedule this additional appointment for you.
TB screening
Required for all employees. TB history will be reviewed and further testing may be recommended.
Immune Status Screening
Email your immunization records in advance to [email protected]
if you have documentation of prior vaccination or antibody titers.
Employee Health will review your immune status of measles, mumps, rubella (MMR), varicella (Chickenpox), Hepatitis B, TDAP, and seasonal influenza. Employees who do not have sufficient documentation of immunity may be advised to receive the appropriate vaccination within 30 days of their start date.
*This is a separate appointment from the physical and urine drug test.

Employee Health Procedures by Work Location
On-site, hybrid and fully remote employees
All new hires must complete a physical, drug screen and employee health screening before the first day of employment.
On-Site Employee
Employees working at a Cottage Health location will complete their physical and drug screen at one of our local clinics listed below.
Hybrid Employee
If you are not located near a Cottage Health facility, you may schedule your physical and drug screen at an approved clinic near you. To complete your employee health screening you can work with your TA Business Partner to discuss your options.
Fully Remote Employee
You will receive a list of approved clinics in your area where you can complete your physical and drug screen. Your employee health screening may also be done at an approved clinic, work with your TA Business Partner to confirm details.
Relocating Employee
If you are relocating to the Santa Barbara area, please complete your physical and drug screen at an approved clinic near your current residence before moving. Plan to arrive a few days before your start date to complete your Employee Health screening appointment, which must be completed on a weekday.

**Parking is limited at Cottage Rehabilitation Hospital. Street parking is available in the surrounding neighborhood, please plan ahead and allow extra time to find parking before your appointment.

Employee Onboarding
Steps and resources for on-site, hybrid and remote team members
Complete Assigned Journeys
You will receive a "Welcome to Cottage Health" email with details on how to complete your assigned Journeys online through "Cottage Compass." Please complete within a week of receiving.
License and Certification Verification
All required licenses and certifications need to submitted no later than the Wednesday before your orientation date. (Start date could be impacted if not completed.)
Health Screening
A list of local health care facilities will be provided for you to complete your required pre-employment visit. During this appointment, your physical, drug screen (required for all positions), and mandatory TB screening will be completed.*
Schedule I-9 Verification
On-site and Hybrid employees: complete the I-9 form in your Journeys, and present approved documents at orientation.
Remote employees - an approved third-party vendor will be in contact to complete the I-9 paperwork.
Equipment delivery
Receive your laptop and materials by FedEx (hybrid and remote employees only)
IT support
You will receive an email with the details to log in and set up your account.
*Depending on your role, additional services may include blood sample collection and/or fit testing.
